Operations and Maintenance Forms

There are three forms for septic operations and maintenance (O&M). There are links to these three forms on the link bar to the right. All three of these forms can be filled out and saved on your computer.  Please, read below for more information on these forms.

Homeowners Guide to Completing Operations and Maintenance Inspections:

This guide will assist homeowners in completing their O&M inspection and filling out the form.  It contains instructions for every step of the O&M inspection process and instructions for filling out the form.

O&M Standard form:

The "O&M Standard Form" will work for the majority of septic systems in Cowlitz County.  This form is for gravity systems, pump to gravity systems, and pressure distribution systems.

O&M Sand Media form:

The "O&M Sand Media" form is for septics that utilize sand lined trenches or beds, bottomless sand filters, or sand mounds.  If you have a sand filter complete this form plus the sand filter supplemental report.

Sand Filter Supplemental Report:

The "Sand Filter Supplemental Report" form is to be filled out in addition to the "O&M Sand Media" form if you have an intermittent sand filter as part of your septic system.
