This is the permit required for the
repair, installation of a replacement, or alteration of any on-site
sewage system. In order to apply for a repair permit, you will
need to submit a completed application form along with the permit fee.

If your repair will involve the replacement of your septic drainfield,
you will also need to work with an on-site septic designer or engineer
to have a proper soils site evaluation and system design completed.

with a design from a
septic designer or engineer. Fee is due with the
application.

on application form:

Applicant's name, address, telephone number:
The applicant is the proposed buyer, developer or owner of the
property. Realtors, sellers, or contractors should have written
permission from the owner before applying.

Property owner: As
recorded in the assessor's office.

Project address: Fill in
road/street name and city.

Legal description: This
information may be obtained from the owner, tax statement, plat maps,
assessor's office, or earnest money agreement.

Lot size: State size of
the entire parcel in acres or square feet.

Describe project:
Indicate the type of dwelling or commercial use of buildings building
on the parcel. If a dwelling, indicate the number of bedrooms in the
space provided. For commercial uses, state the number of people per
day and the nature of business.

Water supply: Indicate
type of water source; public or private, individual well or spring.

Vicinity map: This should be a
small-scale drawing to show the lot in relationship to major roads,
streams, and other permanent landmarks. Show the approximate location
of existing homes with addresses, named roads, driveways, and
intersections with distances from the parcel. This map aids in
locating the parcel in the field.

Plot plan: This is the large-scale map of the
parcel alone. For parcels of 10 acres or more, use a scale of one inch
equals 50 feet (1" = 50'). The entire parcel may not fit in the space
provided. Give the lot dimensions but show the portion of the lot to
be developed in relationship to two (2) adjacent property lines. Use
permanent landmarks as reference points to at least one of the test
holes. For smaller parcels, use a larger scale (please indicate
scale). Indicate the following:

North direction

Property lines

Existing or proposed
buildings, driveways, roads, wells, drainfields, etc.

Downslope direction(s),
if any

If within 200 feet,
show all streams, lakes, ponds, sloughs, swamps, seasonal
drainageways, springs, wells, etc.
