Description
and Mission of the Program
The
Maternal, Child and Family Health program of the Cowlitz County Health
Department provides a number of services to the residents of Cowlitz
County.
The
mission of the program is to promote the health and wellness
of women, children and their families. Community partners include:
Planned
Parenthood, Parents Place, Family Health Center, Head Start, Peace
Health and
private health care providers.

What
services are provided by Health Department professionals for Cowlitz County?
-
Public
Health Nurses will do a Work
First evaluation upon referral from DSHS.
-
Provide
education and consultation to families to increase parenting
skills.
-
Facilitate
access to medical, educational, social, psychological and
support services for families at risk.

Referrals
to state funded health insurance programs.
-
Consultation
and assessment by a public health nurse of children with special
health care needs, pregnant women and their families.

Family
planning services referrals.
-
Integrated
Maternity Support Services for pregnant women, parenting support
for parents of newborns/infants, childbirth education, child
care while at medical appointments and transportation to medical
appointments.

Collaboration
with social workers and/or foster parents to develop and
implement preventative, treatment, therapeutic and /or
rehabilitative measures for children in foster care more than 90
days and children with special health care needs.

Referrals
to private health care providers for diagnosis and treatment of
medical problems for children, expecting mothers and their
families.
